Community nursing care is available for services that are related to your disability and contribute to your long-term wellbeing. Our nurses can provide support and clinical interventions to meet your healthcare needs. They can also provide training to enable you, a family member, friend or support worker to undertake supports so that these can be achieved without nursing assistance. The types of service our nurses provide includes:
- Health care assessments including continence assessments
- Clinical interventions such as woundcare, catheter and PEG changes
- Health related training such as medication prompting and wound care
- Support to access to a GP or other health professional
